Overview
My Aged Care coordinates access to government-funded aged care services, including home modifications that help seniors live safely and independently. Support is available through the Commonwealth Home Support Programme (CHSP) and Home Care Packages.
Eligibility Requirements
- You must be 65 years or older (50+ for Aboriginal and Torres Strait Islander people)
- You need help to continue living independently at home
- You are an Australian citizen or permanent resident
- You are living in Australia

The Process

Call 1800 200 422 to register and discuss your needs. Have your Medicare and DVA numbers ready.
Initial Assessment
A phone assessment determines what level of support you might need.
Face-to-Face Assessment
If needed, an assessor visits your home to understand your needs in detail.
Approval and Referral
You'll be approved for either CHSP services or a Home Care Package based on your needs.
Choose Your Provider
Select from approved service providers in your area.
Services Begin
Your chosen provider arranges and completes the modifications.
